Media Waiver
I grant SAMs: Shaping Academic Minds, LLC dba TagTeam Tutoring permission to all the foregoing to use any photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose.
Insufficient Funds / Declined Payments
All sessions are paid prior to receiving services. Insufficient charges will incur a $30 fee. Sessions will be canceled until payment is received.
Liability Behavior Waiver
My child(ren) are of acceptable behavior and medically able and properly trained to be a participant. My children will abide by all decisions of adult supervision.
Liability Risk Waiver
I assume all risks associated with SAMs: Shaping Academic Minds, LLC dba TagTeam Tutoring, including but not limited to falls, contact with other participants, the effects of the weather, all such risks being known.
Liability Release Waiver
Having read this waiver and knowing these facts and in consideration of your accepting my form to participate in the tutoring program, I, for myself and anyone entitled to act on my behalf, waive and release SAMs: Shaping Academic Minds, LLC dba TagTeam Tutoring and its officers and agents, all sponsors, their representatives and successors from all claims or liabilities even though liability may arise out of negligence or carelessness on the part of the persons named in this waiver.
Liability Loss Waiver
I understand that SAMs: Shaping Academic Minds, LLC dba TagTeam Tutoring and its officers and agents, all sponsors, their representatives and successors is not responsible for lost items during tutoring services.
Refund Policy
There will be no refunds given. Overpayments will be applied to your account. This includes cancellations.
